There are several good suggestions in there. I will check in with some of the other groups and see which of them are planning surveys and when.
Now, for your second suggestion, widely disseminating the survey. It is a valuable idea, but we should probably define a bit more about who it is we would be trying to survey in that case. So far, I had been thinking of this as a survey of the NDSA membership. In this case, the idea would be that we can make and substantiate claims about member organizations.
The nice thing about doing it just inside the NDSA is that we know how many members there are, so we can very easily calculate a response rate, which makes claims about the memberships approaches more potent.
With that said, that kind of focus would not speak to some more general community of organizations working on digital stewardship.
TLDR; so far we have been talking about NDSA member approaches. So do we want to survey the membership, or do we want to survey a broader group? If we do want to survey a broader group how would we bound it and how would we identify response rates?

Sorry, I won't be able to attend today. I'm supportive and, most often, watching through emails, but have some conflicts around many of the meeting times.
Anyway, I like the thoughts/ideas presented below. My only comment is on the survey tool. I think our survey needs to be coordinated with the other groups, not sent to them. Then, I think 'the survey' should be well publicized and go out to various stakeholder organizations. For example, I could provide a listserv for the national GISers.
I think we need simple questions (as you suggest - buttons, Likert scale numbers) and delivery to archivists, coordinators and, most importantly, the primary custodians/providers.
Surveys are everywhere these days. To keep this one from getting ignored, It shouldn't be one of six going out from the NSDA groups, it should be sent on their behalf, and with high importance, from LoC.
That's my two cents.

Just a quick reminder about the call today at 1pm. After we chat a little about the workshops and the meeting, as I see it, these are the next three things to talk through.
1. Next steps with our document.
I and a few of the team members can share it at LC's meeting with storage vendors in the September. What do we want to have done for this to share it with them?
2. Decision tree?
On an earlier call, Micah had volunteered to draft up a decision tree dealing with some of the issues that emerged from the questionnaire. We got requests for something like that at the workshop. Micah, is there any more work you want to see before you take a crack at drafting a decision tree?
3. Should we do a more structured survey of the full membership?
Do we want to create a more targeted survey of the membership based off of some of the issues that emerged in the questionnaire. Think radio buttons and numerical answers. Do we want this kind of information about some of our questions? If so, which questions? I'm willing to draft a short survey and share it with the list for input. Does that sound useful?
