Request for Information Preserve Digital Ohio Custom WordPress Form Project Introduction This document is a Request for Information (RFI) regarding vendor capabilities in creating a custom WordPress form for the Preserve Digital Ohio (PDO) website. The PDO team is seeking a contractor to create a custom form that will guide users through a questionnaire and insert user-submitted responses into a spreadsheet template that the user will then be able to download via the frontend, preferably in Microsoft Excel format. The data will also be collected in a backend database. The questionnaire should be optimized for usability. Preserve Digital Ohio is a project team that aims to assist small cultural heritage organizations in intellectual and digital preservation of their born-digital and digitized collections. The PDO website will be launched in early October and is a tool that will assist organizations in this effort.
