Koha uses Coce, a cover image cache that pulls them from Google Books, Open
Library, and Amazon (requires credentials):

I can't necessarily say how well it works at scale, it's fine for us but we
are a small (<2000 FTE) school.In the past I had issues with this service
because it would send mixed HTTP and HTTPS URLs but it seems like that's
been resolved and it only sends secure resources now.
